Using Metformin for Type 1 DM is a bit controversial. We do not use Metformin as standard therapy in Type 1 DM. However in those with Type 1 DM who are overweight and /or who require large doses of insulin (more than 1.5units per kg body weight), one can try Metformin because we know that Metformin reduces insulin resistance (those who are overweight and those who require large doses of insulin are known to have insulin resistance). I have a patient who is a T1DM that was given insulin and metformin as treatment. It is said that metformin can help reduce the dose of insulin required if given as a combination with insulin for T1DM, how does that work ?
